Basic Life Support BLS (Healthcare Provider CPR)

• Credential card valid for 2 years.
• Class fee is $60 or $50 for students with valid school ID card.
• Online payment is NOT available. Credit card payment is due at the time of class
  
• Manual $25 (optional).
• For healthcare providers & allied health professionals.
• Infant/child/adult CPR & AED training is included.
• Skills testing and multiple choice written exam.

The class is held in the Education Classroom at OSU Medical Center,

2nd Floor of main hospital, 744 W. 9th St., Tulsa, OK 74127.

DIRECTIONS:

• 11th & Houston, downtown, just off the Broken Arrow Expressway 
• westbound.
• Park in the parking garage: visitors may park on Level one or two.
• Enter MAIN HOSPITAL through East Lobby Entrance.
• Turn right at the information desk and follow signs to the north elevators.
• Take north elevators to the 2nd floor and turn right.
• Follow signs for the "Nursing Logistics" department (near the auditorium). 
• The education classroom is in the Nursing Logistics department.

3. Food is not provided. Please bring snacks, if needed, or you can purchase

them from the vending machines.
